Together with the form, you will find a dental chart which you can use as a guide to assist you in specifying the exact location of each tooth and its corresponding number.

Step 2: OBTAIN AN OPG X-RAY |
For Dr. Biju Thomas to provide the treatment plan that will best meet your dental needs, he will require an OPG X-ray to be taken and sent to him.
Contact the dental clinic or hospital nearest you and ask if they have OPG X-ray facility available and how you can have it taken there. Get the diagnostic procedure done and once you have the result, send it us.
Your options in sending us the OPG X-ray result are as follows:
- Drop it at the clinic

Send it by mail to P.O. Box 84686, Dubai, U.A.E.
- Send it by email.
This can be done by scanning the result and saving it as a small jpg format file. Then, attach the file to your email and send it to info@dubaidentalimplantclinic.com.
Upon receiving the OPG X-ray result, Dr. Biju Thomas will send you your personal plan of treatment and the estimated treatment cost for your consideration.

Step 3: SCHEDULE AN APPOINTMENT WITH DR. THOMAS |
